Between 1993 and 1995 we lost two VLPC characterization cassettes (for use in the Oxford flow cryostats) to overheating due to warmup heater runaway, and most recently (fall of 1998) one VLPC characterization cryostat (for the Boeing test stand clone) in exactly the same way.
Update January 2002: AFE board 560619 suffered a local 3rd degree burn (a nice black hole where one of the multiplexers for module 2 used to be) while on the Phase V test stand; no VLPCs or cassettes were harmed in the incident....
