IP Address Request Form

The following information is required to process your request for a new IP Address at D0. The information in this form is emailed to the D0 Net Manager mailing list (d0-netmgr@fnal.gov), who then applies for the address with the Computing Division.

All of the information is required to process the request.

The nodename of your Windows PC will be based on the operating system and your institution.
EXAMPLE: A Brown University NT machine would be D0NTBRxx.FNAL.GOV.
This convention is required to support the large number of PC's at the experiment. Sorry, there are no exceptions.

Linux node "static" IP addresses are not assigned using this form. Please contact the Clue-D0 administrators for help with your Linux node. Linux laptop DHCP registration should be done with this form.
